KNOW ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that we the undersigned Nigel McClarey, Mechanic and Janet O’Kane, housewife of 303 Clooney Road, Limavady, County Londonderry BT49 9JE, the legal guardians of Jamie Lee Kane McClarey, a minor born on 15th March 1999 now or lately called Jamie Lee O’Kane-McClarey a Commonwealth Citizen, on behalf of the said Jamie Lee Kane McClarey.

WITNESS AND IT IS HEREBY DECLARED as follows:­

On behalf of Jamie Lee Kane McClarey we absolutely and entirely renounce relinquish and abandon the use of his former name of Jamie Lee O’Kane-McClarey and assume adopt and determine to take and use the name of Jamie Lee Kane McClarey in substitution for his former name of Jamie Lee O’Kane-McClarey. That the said Jamie Lee O’Kane-McClarey shall at all times hereafter in all records, deeds and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Jamie Lee Kane McClarey in substitution for his former name of Jamie Lee O’Kane-McClarey so relinquished as aforesaid to the intent that he may hereafter be called, known or distinguished not by the former name of Jamie Lee O’Kane-McClarey but by the name of Jamie Lee Kane McClarey only.

We hereby on behalf of the said Jamie Lee Kane McClarey authorise and require all persons at all times to designate describe and address him, by the adopted name of Jamie Lee Kane McClarey.
