Known all men by these presents (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Barbara Una Vallely of 371 Ballyoran Park, Portadown in the County of Armagh, single, student now or lately called Barbara Una Doran a commonwealth citizen.

Witnesseth and it is declared as follows:

1.	I absolutely and entirely renounce relinguish and abandon the use of my said former surname of Doran and assume, adopt and determine to take and use the surname of Vallely in substitution for my former surname of Doran.
2.	I shall at all time herein an all records, deeds, documents and other writings and in all actions and proceedings as well as in all dealings and transactions and all on occasions whatsoever use and subscribe the said name of Vallely as my surname in substitution for my former surname of Doran so relinquished as aforesaid to the intent that I may hereinafter be called, known or distinguished not by the former surname of Doran but by Vallely only.
3.	I authorise and require all persons at all times to designate, describe and address me to the adopted surname of Vallely.

In witness whereof I have hereunto subscribed my Christian or first names of Barbara Una and my adopted and substituted surname of Vallely and also my said former surname of Doran and have affixed my seal this 16th day of June, 2003.
