Deed Poll

This change of Name Deed is made this 20th day of October 2003 by me the undersigned formerly called Mary Ledgerwood McAvoy.

1.	I declare that I have absolutely relinquished and abandoned the use of my former middle name of Ledgerwood and in substitution therefor I have adopted and hereby formally assume the middle name of Caitlin.
2.	I intend at all times hereafter to use only the names Mary Caitlin McAvoy and hereby authorise all persons to designate and address me accordingly.
