Know all men by these presents (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Jacqueline Eileen Smith of 36 Broomhill, Portadown, Craigavon in the County of Armagh, Married Woman, being a Care Assistant and the legal guardian of Nicola Smith whose date of birth is 24th February 1988, now or lately called Nicola Steele a Commonwealth Citizen on behalf of the said Nicola Smith.

Witness and it is hereby declared as follows:-

On behalf of Nicola Smith I absolutely and entirely renounce, relinquish and abandon the use of her former surname of Steele and on her behalf assume, adopt and determine to take and use the surname of Smith in substitution for her former surname of Steele. That the said Nicola Smith shall at all times hereafter in all records, deeds and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Smith as her surname in substitution for her former surname of Steele so relinquished as aforesaid to the intent that she may hereafter be called, known or distinguished not by the former surname of Steele but by the surname of Smith only.

I hereby on behalf of the said Nicola Smith authorise and require all persons at all times to designate, describe and address her by the adopted surname Smith.
