Know All Men By These Presents (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Thomas Patrick Ireland of 15 Pinewood View, Newcastle, Co. Down BT33 0HF, single part time Sales Person now or lately called Thomas Patrick Bell a commonwealth Citizen.
Witness And It Is Hereby Declared as follows:
I absolutely and entirely renounce, relinquish and abandon the use of my said former Surname of Bell and my said former Christian name of Thomas, and assume, adopt and determine to take and use the Surname of Ireland and the Christian name of Tomás in substitution for my former Surname of Bell and the Christian name of Thomas.
I shall at all times hereafter in all records, deeds, and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and all occasions whatsoever use and subscribe the said name of Ireland as my Surname in substitution for my former Surname of Bell and the Christian names of Tomás Patrick in substitution for my former Christian names of Thomas Patrick so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former Surname of Bell by the Surname of Ireland only, and the former Christian names of Thomas Patrick but by the Christian names of Tomás Patrick only.
I authorise and require all persons at all times to designate, describe and address me by the adopted Surname of Ireland and the adopted Christian name of Tomás.
In Witness Whereof I have hereunto subscribed my former Christian or first name Thomas and my adopted and substituted Christian name of Tomás and my adopted and substituted surname of Ireland and also my said former surname of Bell and have affixed my seal this 8th day of January 2004.
