Know All Men By These Presents (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Lisa Dianne Mary Ward of 182 Lagmore Meadows, Stewartstown Road, Dunmurry BT17 0TJ. I am single and am unemployed, now or lately called Lisa Dianne Mary Carson a Commonwealth Citizen.

Witnesseth And It Is Hereby Declared as follows:

I absolutely and entirely renounce relinquish and abandon the use of my said former surname of Carson and assume adopt and determine to take and use the surname of Lisa Dianne Mary Ward in substitution for my former surname of Carson.

I shall at all times hereafter in all records, deeds and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Ward as my surname in substitution for my former surname of Carson so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former surname of Carson but by the surname of Ward only.

In Witness whereof I have hereunto subscribed my former name of Lisa Dianne Mary Carson and my new and assumed name of Lisa Dianne Mary Ward and affixed by me sealed the day and year first above written.
