Deed Poll on Change of Name

THIS CHANGE OF NAME DEED (intended to be enrolled at the Supreme Court of Judicature in Northern Ireland) made this day of 11th June 2004 by me the undersigned:-

Of 5 New Bridge Street, Downpatrick, County Down, Northern Ireland, BT30 6EY, now or lately called Richard Michael Steven Lever, a British Citizen under Section 11 of the British Nationality Act 1981.

WITNESSES AND IT IS HEREBY DECLARED as follows:

1. I absolutely and entirely renounce relinquish and abandon the use of my former surname of Lever and assume adopt and determine to take and use from the date hereof, the surname of McIlheron in substitution for my former surname of Lever.
2. I shall at all times hereinafter in all records, deeds, documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of McIlheron as my surname in substitution of my former name of Lever so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former surname of Lever but by the surname of McIlheron only.
3. I authorise and require all persons at all times to designate describe and address me by the adopted surname of McIlheron.

IN WITNESS whereof I have hereunto subscribed my Christian or first name or names of Richard Michael Steven Lever and my adopted and substituted surname of McIlheron.
