Change of Name Deed

I, Eleanor Larkin of 10 Sandybraes, Magherafelt, in the County of Londonderry hereby give Notice that by a Change of Name Deed dated 10 June 2004, I have absolutely relinquished and abandoned the use of my former surname of O’Boyle and I hereby authorise all persons to designate and address me by the surname of Larkin only.
