Notice is hereby given that by a Deed Poll dated 7th July 2004 and enrolled in the Supreme Court of Judicature in Northern Ireland on the 23rd August 2004, Rhonda Patricia Majella McCullagh as having the parental responsibility on behalf of Jordan Lee McCullagh, a child, a single and a Commonwealth Citizen abandoned the surname of Farren and assumed in lieu thereof the surname of McCullagh.
