KNOW ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central Office of the High Court of Justice In Northern Ireland) that I the undersigned Janet Stewart of 1 Ballyharry Heights, Newtownards, Single Proprietor Residential Home, now or lately called Janet Todd, a Commonwealth Citizen.

WITNESSETH AND IT IS HEREBY DECLARED as follows:

I absolutely and entirely renounce relinquish and abandon the use of my said former surname of Todd and assume adopt and determine to take and use the surname of Stewart in substitution for my former surname of Todd.

I shall at all times hereafter in all records, deeds and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Stewart as my surname in substitution for my former surname of Todd so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former surname of Todd but by the surname of Stewart only.

I authorize and require all persons at all times to designate describe and address me by the adopted surname of Stewart.

In Witness whereof I have hereunto subscribed my Christian or first Name of Janet and my adopted and substituted surname of Stewart and also my said former surname of Todd and have affixed my seal this 23rd day of September 2004.
