KNOW ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Christopher James McAllister of 12 Ridgeway Drive, Single man, Sales Assistant now or lately called Christopher James Young a Commonwealth Citizen

WITNESSETH AND IT IS HEREBY DECLARED as follows:–

I absolutely and entirely renounce relinquish and abandon the use of my said former surname of Christopher James Young and assume adopt and determine to take and use the surname of Christopher James McAllister in substitution for my former Surname of Young.

I shall at all times hereafter in all records, deeds, and documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name Christopher James McAllister as my surname in substitution for my former surname of Christopher James Young so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former surname of Christopher James Young but by the surname of Christopher James McAllister only.

I authorise and require all persons at all times to designate describe and address me, by the adopted surname of Christopher James McAllister.

IN WITNESS whereof I have hereunto subscribed my Christian or first names of Christopher James and my adopted and substituted surname of McAllister and also my said former surname of Young and have affixed my seal this
