KNOW ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central office of the High Court Of Justice of Northern Ireland) that I the undersigned Kerry Ann Stinson of 27 Tulnavern Road, Ballygawley, in the County of Tyrone, single, student, now or lately called Kerry Ann Hawkins, a Commonwealth citizen.

WITNESSES AND IS HEREBY DECLARED as follows

I ABSOLUTELY and entirely renounce relinquish and abandon the use of my said former surname of Hawkins and assume adopt and determine to take and use from the date hereof the name of Stinson in substitution for my former name of Hawkins.

I SHALL at all times hereinafter in all records documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Stinson as my name in substitution for my former name of Hawkins so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not by the former surname of Hawkins but by my new surname of Stinson only.

I AUTHORISE and require all persons at all times to designate and address me by the adopted surname of Stinson.

IN WITNESS whereof I have hereunto subscribed my Christian or first name of Kerry Ann and my adopted and substituted surname of Stinson and also my said former surname of Hawkins and have affixed my seal this 13th day of May 2005.
