KNOW ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Thomas James Cunningham of 71 Tildarg Street, Belfast, BT6 8NJ, Single, Security Officer, now or lately called Thomas James Ferguson, a Commonwealth Citizen.

WITNESS AND IT IS HEREBY DECLARED as follows:

1	I absolutely and entirely renounce, relinquish and abandon the use of my said former surname of Ferguson and assume, adopt and determine to take and use the surname of Cunningham in substitution for my former surname of Ferguson.
2	I shall at all time hereafter in all records, deeds, documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name of Cunningham as my surname in substitution for my former surname of Ferguson so relinquished as aforesaid to the intent that I may hereafter be called, known or distinguished not be the former surname of Ferguson, but by the surname of Cunningham only.
3	I authorise and require all persons at all time to designate, describe and address me by the adopted surname of Cunningham.

IN WITNESS whereof I have hereunto subscribed my Christian for first name of Thomas James and my adopted and substituted surname of Cunningham and also said former surname of Ferguson and have affixed my seal this 19th day of August 2004.
