KNOW ALL MEN BY THESE PRESENTS (which are intended to be enrolled at the Central Office of the High Court of Justice in Northern Ireland) that I the undersigned Susan Margaret Groves of 16 Bramblewood Grove, Castlewellan Road, Banbridge in the County of Down, divorced, senior QA Technician now or lately called Susan Margaret Mather a commonwealth citizen.

WITNESSETH AND IT IS HEREBY DECLARED AS FOLLOWS:

1.	I absolutely and entirely renounce relinquish and abandon the use of my said former surname of Mather and assume, adopt and determine to take and use the surname of Groves in substitution for my former surname of Mather
2.	I shall at all times herein in all records, deeds, documents and other writings and in all actions and proceedings as well as in all dealings and transactions and on all occasions whatsoever use and subscribe the said name Groves as my surname in substitution for my former surname of Mather so relinquished as aforesaid to the intent that I may hereafter be called known or distinguished not by the former surname of Mather but by Groves only.
3.	I authorise and require all persons at all times to designate, describe and address me to the adopted surname of Groves.
