I hereby give notice that by a Deed Poll dated 26th November 1997 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A 2LL, on 8th April 1998, I, Karen Jane Clarke, of 19 Docklands Avenue, Ingatestone, Essex, the mother of SAMUEL MILES CLARKE, an unmarried child and a British citizen by virtue of section 11(1) of the British Nationality Act 1981, renounced, relinquished and abandoned the name of Samuel Miles Wilson and assumed the name of Samuel Miles Clarke.
