Notice is hereby given, that I, BARTHOLOMEW SEYMOUR BALCH DIGBY, of Kenelm House, Kingscote, Gloucestershire, formerly called and known by the name of Edward Bartholomew Seymour Balch, a citizen of the United Kingdom by birth, give public notice that on 11th May 1999, I formally and absolutely renounced and abandoned the use of my surname of Balch and assumed and adopted in place thereof the surname of Digby, and furthermore I formally and absolutely renounced and abandoned the use of my first name Edward and assumed and adopted the first name Balch, and further that such change of name is evidenced by a Deed Poll dated 11th May 1999, under the hand of my mother Patricia Mary Digby duly executed by my mother and attested and enrolled in the Enrollment Department of the Central Office of the Supreme Court of Judicature on 2nd July 1999.
