Notice is hereby given that by Deed Poll dated 14th April 1999, and enrolled in the Supreme Court of England and Wales on 23rd June 1999, JANE LOUISA MARIA WATERFALL, of 19 g Kingswood Road, London, a British Citizen by virtue of section 11(1) of the British Nationality Act 1981, abandon the surname of Mugliston, and assume and take thereof my new name Jane Louisa Maria Waterfall.
