Notice is hereby given that by a Deed Poll dated 24th May 1999, and enrolled in the Supreme Court of Judicature on 14th June 1999, I, Amanda Elizabeth Ardley, as a person having parental responsibility on behalf of LEE ARDLEY, a child, single and a British citizen, abandoned the surname of Cocker and assumed in lieu thereof the surname of Ardley.
