Notice is hereby given that I, Carol Ann Clark, Mother and Legal Guardian of ANGIE MICHELLE CLARK, of 16 Hairpin Croft, Peacehaven, East Sussex, formally called and known by the name ANGIE MICHELLE BARROW, a citizen of the United Kingdom by birth, give public notice that on 24th June 1999, I formally and absolutely renounced and abandoned on behalf of ANGIE MICHELLE CLARK the use of her surname of BARROW and assumed and adopted in place thereof the surname of Clark and further that such change of name is evidenced by a a deed poll dated 24th June 1999 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 29th July1999.
