I hereby give notice that by a Deed Poll dated 8th May 1999 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A 2LL, on 4th October 1999, I, Caroline Marie Cook, of 17 Willow Way, Guildford, Surrey GU1 1PF, the mother of SOPHIA TYLER COOK-BROWNE, an unmarried child and a British citizen, renounced, relinquished and abandoned the name of Sophia Tyler Browne and assumed the name of Sophia Tyler Cook-Browne.
