Notice is now given that by a Deed Poll dated 18th October 1999 and enrolled in the Central Office of the Supreme Court on 1st November 1999, ELAINE SAMANTHA MITCHELL, of 12 Bolingbroke Close, St. Dials Wood, Monmouth, Monmouthshire NP25 5UA, single, a British citizen under the British Nationality Act 1981 section 11(1) formally and absolutely renounced and abandoned the surname of Lewis and has assumed and adopted and intends on all occasions to use and subscribe the surname of Mitchell instead of the surname of Lewis so as to be at all times called known and described by the name of Elaine Samantha Mitchell exclusively.
