Notice is hereby given that I, Emma McEvoy, of 13 Moorfields Road, Liverepool, on behalf of LYDIA McEVOY, formerly called and known by the name of Lydia Fox, a citizen of the United Kingdom by birth, given public notice that, on 7th March 2000, I formally and absolutely renounced and abandoned the use of her surname of Lydia Fox and assumed and adopted in place thereof the surname of Lydia McEvoy and further that such change of name is evidenced by a Deed Poll, dated 7th March 2000, under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 27th March 2000.
