Notice is now given that, by a Deed Poll dated 5th October 2000 and enrolled in the Central Office of the Supreme Court on 23rd October 2000, I, CHRISTOPHER JOHN ELLIS, of 30 Morston Close, Tadworth, Surrey KT20 5JN, married and a British citizen, under the British Nationality Act 1981, section 11(1), formally and absolutely renounced and abandoned the surname of Pratt and have assumed and adopted and intend on all occasions to use and subscribe the surname of Ellis instead of the surname of Pratt so as to be at all times called, known and described by the name of Christopher John Ellis exclusively.
