INSOLVENCY ACT 1986, SECTIONS 392 AND 393

Take note that I, Anthony Hornshaw, Official Receiver, of 25 Queen Street, Leeds LS1 2TW, intend to apply under the provisions of the above Act for authorisation to act as an Insolvency Practitioner.
