Notice is hereby given that I, SHAN MYLVAGANAM, of 99 Carr Road, Northolt, Middlesex UB5 4RB, formerly called and known by the name of Mylvaganam Shanthsmoorthy, a Commonwealth citizen, as defined in section 37(i) of the British Nationality Act 1981, give public notice that, on 12th January 2001, I formally and absolutely renounced and abandoned the use of my name of Mylvaganam Shanthsmoorthy and assumed and adopted in place thereof the name of Shan Mylvaganam and further that such change of name is evidenced by a Deed Poll dated 12th January 2001 under my hand duly executed by me and due to be attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature.
