Notice is hereby given that I, ANN MARIE DAWN DE VEER, of 26 Broadgate Road, Sutton St. James, Lincolnshire, formerly called and known by the name of Adrian Paul Dunnett, a British citizen of the United Kingdom by birth, give public notice that, on 3rd September 1997, I formally and absolutely renounced and abandoned the use of my name Adrian Paul Dunnett and further that such change of my name is evidenced by a Deed Poll dated 3rd September 1997 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 16th March 2001.
