Notice is now given that by a Deed Poll dated 21st May 2001 and enrolled in the Central Office of the Supreme Court on 6th June 2001, I, MUSASHI STEPHEN WILLIAMS, of 186 a Gloucester Terrace, London W2 6HT, married and a British citizen, under the British Nationality Act 1981, section 1, formally and absolutely renounced and abandoned the forename of Jeremy and have assumed and adopted and intend on all occasions to use and subscribe the forename of Musashi and the surname of Williams instead of the forename and surname of Jeremy Williams so as to be at all times called, known and described by the name of Musashi Stephen Williams exclusively.
