Notice is hereby given that MICHAEL JOHN EVERETT, of 6 Collard Avenue, Loughton, Essex, formerly called and known by the name of Michael John Furzer, a citizen of the United Kingdom by birth, gives public notice that, on 13 June 2001, he formally and absolutely renounced and abandoned the use of his surname of Furzer and assumed and adopted in place thereof the surname of Everett and further that such change of name is evidenced by a Deed Poll dated 13 June 2001 under his hand duly executed by him and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 12 July 2001.
