Notice is hereby given that by a Deed Poll dated 26 June 2001 and enrolled in the Supreme Court of Judicature on 28 September 2001, Tracy Joan Hampson as a person having parental responsibility, on behalf of ELEANOR JASMINE HAMPSON, of 6 Bowland Close, Carnforth, Lancashire LA5 9UP, a child, single and a British citizen, abandoned the surname of Emmott and assumed the surname of Hampson.
