Notice is now given that by a Deed Poll dated 5 February 2002 and enrolled in the Central Office of the Supreme Court on 5 February 2002, I, SARA BOOKER KEMPE, of 106 Sydney Road, Muswell Hill, London N10 2RN, married and a British citizen, under the British Nationality Act 1981, section 11, formally and absolutely renounced and abandoned the forename of Amanda and assumed and adopted and intend on all occasions to use and subscribe the forename of Booker instead of the forename of Amanda so as to be at all times called, known and described by the name of Sara Booker Kempe exclusively.
