Notice is hereby given that by a Deed Poll dated 12 July 2002 and enrolled in the Supreme Court of Judicature on 19 July 2002 by Sheila Ann Perry as the person having parental responsibility, on behalf of DANIEL FRANCIS PERRY, a child, single and a British citizen, under section 11(1) who has also signed the Deed Poll being of 17 years, abandoned the surname of McCarthy and assumed in lieu thereof the surname of Perry.
