Notice is hereby given that I, CARL MICHAEL MUSCROFT, of 119 Westgate, Pickering, North Yorkshire YO18 8BB, formerly called and known by the name of Carl Michael Rushworth, single and a British citizen, by virtue of section 11(1)(a) of the British Nationality Act 1981, give public notice that, on 10 March 2003, I formally and absolutely renounced and abandoned the use of my surname of Rushworth and assumed and adopted in place thereof the surname of Muscroft and further that such change of name is evidenced by a Deed Poll dated 10 March 2003 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 11 April 2003.
