Notice is hereby given that I, SARAH DAWN MARY BARNARD-EDWARDS, of 27 Curlys Way, Swallowfield, Berkshire RG7 1QZ, formerly called and known by the name of Sarah Dawn Mary Edwards, a citizen of the United Kingdom by birth, give public notice that, on 1 May 2003, I formally and absolutely renounced and abandoned the use of my surname of Edwards and assumed and adopted in place thereof the surname of Barnard-Edwards and further that such change of name is evidenced by a Deed Poll dated 1 May 2003 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 27 November 2003.
