Notice is hereby given that by a Deed Poll dated 15 October 2003 and enrolled in the Supreme Court of England and Wales on 10 December 2003, I, UPALI GAJANAYAKE, of 7 Highfield Avenue, Kingsbury, London NW9 0PD, married and a Commonwealth citizen, by virtue of section 37(1) of the British Nationality Act 1981, abandoned the name of Upasena Gajanayake and assumed in lieu thereof the name of Upali Gajanayake.
