Notice is hereby given that by a Deed Poll dated 29 January 2004 and enrolled in the Central Office of the Supreme Court of Judicature on 2 February 2004, I, Cindy Ann Hockley, as person having parental responsibility, on behalf of the said PAUL ANTHONY HOCKLEY, who is a child, single and a Commonwealth/British citizen, under section 1(1) of the British Nationality Act 1981, abandoned the name of Paul Anthony Baccarini and assumed the name of Paul Anthony Hockley.
