Notice is now given that by a Deed Poll dated 6 March 2004 and enrolled in the Central Office of the Supreme Court on 26 April 2004, CARRIE ANNE BUJOK, of 17 Pen y Bryn, Brecon Road, Builth Wells LD2, single and a British citizen, under the British Nationality Act 1981, section 11(1), formally and absolutely renounced and abandoned the surname of Webb and has assumed and adopted and intends on all occasions to use and subscribe the surname of Bujok instead of the surname of Webb so as to be at all times called, known and described by the name of Carrie Ann Bujok exclusively.
