Notice is hereby given that by a Deed Poll dated 12 December 2003 and enrolled in
the Supreme Court of Judicature on 16 June 2004, KAMRAN AZAM, of 47 Lake Avenue, Slough
SL1 3BY, single and a British citizen, under section 11(1) of the British Nationality
Act 1981, abandoned the name of Jaspreet Singh Talwar and assumed the name of Kamran
Azam.
