Notice is hereby given that I, Nicola Clair Macer, of 43 Orchard Way, Breachwood Green, Hertfordshire SG4 8NT, formerly Nicola Clair Reeve, a citizen of the United Kingdom by birth give public notice that on 10 June 2004, I on behalf of my daughter GEORGIA LOUISE REEVE MACER, formally and absolutely renounced and abandoned the use of her forenames Georgia Louise and her surname Reeve and assumed and adopted in place thereof the forenames of Georgia Louise Reeve and the surname of Macer and further that such change of name is evidenced by a Deed Poll dated 10 June 2004, under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 24 June 2004.
