Notice is hereby given that I, RICHARD O’SULLIVAN, of Flat 3, Norwood Court, Norwood Road, Stretford, Manchester M32 8PL, formerly called and known by the name of Richard Butterfield, a citizen of the United Kingdom by birth, give public notice that, on 25 June 2004, I formally and absolutely renounced and abandoned the use of my surname of Butterfield and assumed and adopted in place thereof the surname of O’Sullivan and further that such change of name is evidenced by a Deed Poll dated 25 June 2004 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 16 December 2004.
