Notice is hereby given that by a Deed Poll dated 25 August 2005 and enrolled in the Supreme Court of Judicature on 13 October 2005, Tracey Sarah Medlyn, of 55 Trenoweth Estate, North Country, Redruth, Cornwall TR16 4AH, as a person having parental responsibility for DANIELLE LOUISE CLAIRE MEDLYN, a child being single, 7 years old and a British citizen, under section 1(1) of the British Nationality Act 1981, abandoned the surname of Blanks and assumed in lieu thereof the surname of Medlyn.
