Notice is hereby given that a Deed Poll dated 20 October 2005, and enrolled in the Supreme Court of Judicature on 8 November 2005, Patience Ese Okuoimose, of 26 Somerfield Street, Rotherhithe, London SE16 2RT, as person having parental responsibility on behalf of the said Racheal Omoyemi Olofinbinu, who is a child, single, a British citizen under section 1(1) of the British Nationality Act 1981 abandoned the name of Racheal Omoyemi Okuoimose and assumed the name of RACHEAL OMOYEMI OLOFINBINU. 20 October 2005.
