Jason Peter Gandy and Marie Frances Gandy as persons having parental responsibility on behalf of the said MATTHEW HENRY GANDY, who is a child, single and a British Citizen under section 1(1) of the British Nationality Act 1981, abandoned the name of Matthew Henry Carruthers and assumed the name of Matthew Henry Gandy.
