Notice is hereby given that a Deed Poll dated 22 June 2006 and enrolled in the Supreme Court of Judicature on 27 June 2006, Helen L Haberfield as person having parental responsibility on behalf of the said JACK LIAM ROBERT HABERFIELD, of 20 Nightingale Way, Bletchingley, Redhill, Surrey RH1 4RF, who is a child single and a British citizen under section 1(1) of the British Nationality Act 1981, abandoned the name of Jack Liam Robert Cross and assumed the name of Jack Liam Robert Haberfield. 22 June 2006.
