I hereby give notice that by a Deed Poll dated 9 December 2005 and enrolled at the Central Office, Royal Courts of Justice, Strand, London WC2A 1LL, I, Kathleen Grace Woodman, 12 Morgan Road, Milton, Portsmouth, Hampshire PO4 8JS, the mother of Daniel Aaron Quaintance Woodman, a child and a British citizen renounced, relinquished and abandoned the name of Daniel Aaron Quaintance Cooke and assumed the name of Daniel Aaron Quaintance Woodman.
