I hereby give notice that by Deed Polls dated 12 October 2007 which have been enrolled at the Central Office, Royal Courts of Justice, Strand, London WC20 1LL, I, Lisa Jane Ball, c/o The Gwyn George Partnership, Solicitors, 110 High Street, Merthyr Tydfil CF47 8AP, the mother of COURTNEY MARIE BALL and JORDAN LEE BALL, British Citizens under section 1(1) of the British Nationality Act 1981, renounce, relinquish and abandon, on their behalf, the surname of Leonard and assumed the surname of Ball.
