Notice is hereby given that a Deed Poll dated 15 February 2008 and enrolled in the Supreme Court of Judicature on 14 April 2008 and as person having parental responsibility on behalf of the said BETHANY LOUISE DAVIES who is a child single and a British Citizen under Section 1(1) of the British Nationality Act 1981 abandoned the name Bethany Louise Chamberlain and assumed the name of Bethany Louise Davies.
