I hereby give notice that by a Deed Poll dated 13 May 2008 and enrolled in the Central Office, Royal Courts of Justice, Strand, London WC2A 1LL on 11 July 2008, I, Trudie Lewis, 1 Tai Ebenezer, High Street, Trelewis CF48 4PG, the mother of Cerwyn James Lewis an unmarried child and a British citizen by birth renounced and relinquished and abandoned the name of Jones and assumed the name of Lewis.
