Notice is hereby given that a Deed Poll dated 9 June 2009 and enrolled in the Supreme Court of Judicature on 19 June 2009, E Carpus, as person having parental responsibility on behalf of the said Jamie Carpus who is a child single and a British citizen under section 1(1) of the British Nationality Act 1981, abandoned the name of Jamie Oliver Joseph Koslover and assumed the name of Jamie Carpus.
