Prakesh Thakorbhai Master-Mistry, of 47 Halstead Gardens, Winchmore Hill, London N21 3DU, is married and a British citizen under section 11(1) of the British Nationality Act 1981, abandoned the name of Prakash Thakorbhai Mistry and assumed the name of Prakesh Thakorbhai Master-Mistry.
