Notice is hereby given that I, Madabushi Lakshmi Madhuri, of 8 Talgarht Walk, West Hendon, London NW9 7HW, formerly called and known by the name of Kidambi Lakshmi Madhuri, a Citizen of the United Kingdom by Naturalisation, give public notice that on 22 July 2010 I formally and absolutely renounced and abandoned the use of my surname of Kidambi, old surname only, and assumed and adopted in place thereof the surname of Madabushi as and further that such change of name is evidenced by a deed poll dated 22 July 2010 under my hand duly executed by me and attested and enrolled in the Enrolment Department of the Central Office of the Supreme Court of Judicature on 22 July 2010.
